1095-c Forms will be mailed by March 2nd
Great News! Salt Lake County is utilizing a new outside vendor to ensure accuracy & compliance on your 2020 1095-c forms.
As a result, all employees will receive a copy of their 2020 1095c in the mail. They will not be available in PeopleSoft.
However, 1095c forms prior to 2020 will continue to be available through Self-Service in PeopleSoft.
The deadline for 2020 1095-c forms is March 2, 2021. If you have not received your form by that date, please contact the HR Benefits team at benefits@slco.org or 385-468-0580 to receive a copy.
HSA ANYTIME
You can now change the Health Savings Account (HSA) deduction amount on your paycheck in PeopleSoft anytime!
Step 1: Log into PeopleSoft https://pshcm.slcounty.org/my.policy
Step 2: Follow this path
Main Menu > Self Service > Benefits > HSA Deduction Update
Step 3: Choose the pay period you want the change to take affect.

Free Counseling for COVID-Related Stress
If you’re struggling to keep up with all the demands, you can talk to a Utah Strong Recovery Project counselor seven days a week from 7 a.m. to 7 p.m. Call/text 385-386-2289 or email your first name and phone number to UtahStrong@utah.gov. Video chat and counseling in other languages are available. This benefit is offered by the State and available to all Utah residents.
If you need an immediate response after hours, you can contact the Utah Statewide Crisis Line at 1-800-273-TALK (8255) or the SAMHSA Distress Hotline at 1-800-985-5990.
