General Information
- Passport applications are being accepted by appointment only.
- Application processing times may vary. Refer to the US Department of State Website for the most up to date processing times.
- Due to mail delays service times for passport processing have increased. Routine service can take up to 13 weeks from the day an application is received at the passport agency. Expedited service can take up to 9 weeks. Please plan accordingly.
Requirements
1. A completed Passport Application (DS-11) form.

5. A certified copy of your birth certificate, if born in the U.S.

6. ALL APPLICANTS must personally appear when applying for a passport.

7. Social security numbers must be provided for all passport applicants. |

* If you are paying for 7 to 9 week service for a passport book you have the option to add $19.53 for Express Mail delivery from the U.S. Department of State. This amount should be included in your check or money order. This option is NOT available for passport cards.
