Home and Community Based Care Provider Information

Salt Lake County Aging and Adult Services invites your company to apply as a provider for The Alternatives Program and the Caregiver Support Program.  These programs directly purchase needed services and supports for clients from a pool of approved providers.  The listing of approved providers is established as a result of contracts from eligible applications received on an ongoing basis.  The Alternatives Program and the Caregiver Support Program are continually looking for opportunities to partner with providers of the following services and supports:

• Adult Day Care
• Assisted Living Facility Respite
• Electric Lift Chairs
• Emergency Response System
• Environmental Adaptation
• Fiscal Intermediary
• Home Health Care
• Medication Reminder System
• Nursing Home Respite
• Personal Care
• Specialized Medical Equipment, Supplies, and Technology Items
