Overview
Information and access to community services and programs
The Information and Assistance Program connects older adults and families to services and resources that help them explore options, assist with independent living, and provide information about health care benefits and fraud prevention.
Services include:
- General information on available services
- Medicare, Medicaid counseling
- Information to help individuals avoid health care fraud
- Assistance with making long term care decisions
- Person centered assessment to create individual care plans
- Short term case management
- Eldercare locator service
Eligibility
Eligibility criteria for these services vary. For more information about service eligibility call 385.468.3200.
To be eligible to receive general information on available services you must:
- There are no eligibility criteria to receive information. Call 385.468.3200 for information Monday – Friday, 8:00 am – 5:00 pm
You must be a Medicare beneficiary to receive Medicaid/Medicare counseling
- To determine Medicaid eligibility call the Department of Workforce Services at 801.526.0950
- To determine Medicare eligibility call Social Security at 1.800.772.1213
To be eligible for Options Counseling you must:
- Be 60 years of age or older
- Homebound
- Need help in decision making and planning for long-term care services
Volunteer
Volunteer Opportunities
Information and Assistance Program volunteers help:
- Conduct home visits with seniors and families to connect them with various services in the community
- Conduct outbound calls to current clients to discuss any changes in situation or needs
- Assist Caseworkers through data entry and community program research
- Conduct follow-up visits/calls to ensure individuals were able to connect with needed services
Volunteer Requirements
To volunteer you must:
- Pass an extensive background check
- Possess a valid Utah driver’s license
- Be accurate in maintaining records and client confidentiality
- Contribute at least 2 hours per week
- Be patient and tactful with the ability to work with diverse populations including those with significant physical and mental impairments
- Possess the desire to help those in need and to learn about various community programs and services
Training and Support
On-the-job training and support is provided by the Volunteer Coordinator and Information and Assistance Caseworkers
