The Salt Lake Valley Health Department (SLVHD) issues birth certificates for births occurring within Salt Lake City limits from 1890 to the present and birth certificates for all of Salt Lake County from 1969 to the present. In addition certificates can be issued for all of Utah for the years 1975 and 1977 through the present. Copies may be obtained by completing an application in person or by requesting a certificate through the mail. The fee is $12 for one certificate and $5 for an additional copy of the same record ordered at the same time. Small wallet size laminated cards are also available for the same fee. Birth certificates are issued only to immediate family members or, if written permission is obtained from an immediate family member, a certificate can be issued to the requestor. - A: First you must be between the ages of 50 and 64 (some exceptions may be made for a woman with an identified breast abnormality).
- Second you must meet the financial eligibility requirement. A woman whose income is 240 percent of the federal poverty level according to household income and family size would qualify.
- Third, make an appointment for a clinical breast exam (CBE) within the SLVHD. A CBE is a physical exam similar to a breast self-exam. The provider will determine the type of mammogram (screening or diagnostic) needed based on the finding of the CBE. The provider will then schedule a mammogram at a participating facility of the woman's choosing and supply her with a payment voucher.

The Environmental Protection Agency (EPA) requires public water systems to be surveyed every three to five years as part of the Safe Drinking Water Act. These are called Sanitary Surveys and include items such as complaints, sampling, cross connections, piping and screening, seals and vents, lids and accesses, disinfections, storage, distribution, pumping, source protection, and waivers to keep drinking water safe and secure. The SLVHD samples drinking water from all County water systems on a regular and random basis to ensure they meet the requirements for safe levels of bacteria.
