Outreach Program
The Outreach Program provides:
- Information and assistance about all of Aging Services programs and community resources for adults age 60 and older, families, professionals, and community service providers.
- In-home assessments of frail seniors to determine their independence needs.
- Meals-On-Wheels eligibility determination.
- Medicare and Medicaid application assistance.
- Training for community organizations.
- There is no eligibility for information and assistance or training for community organizations. For all other services, you must be age 60 and older.
