Alternatives Program

The goal of the Alternatives Program is to enable frail and disabled adults to remain independent for as long as possible.

The Alternatives Program provides in-home assessments to determine services needed, case management and services for frail and disabled adults 18 years of age or older who are at risk of nursing home placement.
