Birth and Death Certificates
Birth certificates are need to obtain a driver’s license, enroll a child in school, apply for a passport and to obtain a death certificate.
Salt Lake Valley Health Department Vital Records offices are conveniently located throughout the Salt Lake Valley and are open Monday through Friday. Receipt of a birth or death certificate is processed while you wait at any of our offices.
